Trans Abstract

Heredity was long considered as a principal cause of epilepsy, even before the advent of the molecular era with Watson and Crick’s discovery of the structure of DNA. However, recent progress in sequencing technology has resulted in the identification of numerous de novo mutations in epilepsy patients, which indicate that epilepsy was not inherited from their parents but newly occurred in the offspring. Thus, the term “genetic” should not be regarded as equivalent to “hereditary,” especially in clinical settings. Meanwhile, epileptologists before the molecular era observed an increased incidence of epilepsy among close relatives of an epilepsy patient. They also found that an organic cause in the brain was usually absent in such cases. The term “idiopathic,” which refers to an internal cause or morbus per se, was used to describe this hereditary predisposition. The term “idiopathic” was adopted in the modern etiologic classification of epilepsy, wherein idiopathic cases are regarded as having a purely genetic etiology, in contrast with cryptogenic or symptomatic cases. Although the International League Against Epilepsy recently proposed changing “idiopathic” to “genetic” in the etiologic classification, the precise genetic mechanism of idiopathic epilepsy has not yet been elucidated. Instead, de novo mutations in new epilepsy-related genes have come from the cryptogenic group.

유전성(Genetic)인가 유전(Hereditary)되는가?

우리 아이, 혹은 제가 앓고 있는 뇌전증이 유전인가요? 진료실에서 흔히 들을 수 있는 표현이지만 의사와 환자에게 각각 다른 의미로 해석되고 이해되는 경우가 많다. 의사는 어떠한 질병이 유전이라고 할 때 특정 유전자의 결함에 의해 그 질환이 발생한다는 의미로 설명을 하게 된다. 이에 반해 환자 혹은 보호자는 유전이라는 의미를 부모 중 한쪽이 특정 질환을 물려주는 경우로 이해하는 경우가 많다. 양측이 이해하고 있는 부분이 완전하게 상충하는 것은 아니지만, 서로 접점을 찾기 위해서는 제법 많은 시간이 필요하게 된다. 이러한 혼란은 “genetic”과 “hereditary”가 우리말로 모두 유전으로 번역되기 때문에 발생한다고 볼 수도 있다.

다음의 경우를 생각해 보자. 6개월 전후에 시작하여 열과 함께 장시간의 발작이 반복되는 드라베 증후군(Dravet syndrome)의 경우 신경계의 기능에 중요한 역할을 하는 소듐 채널 유전자(SCN1A)의 이상에 의해 발생하는 것으로 잘 알려져 있다.1 드라베 증후군 환자에서 발견되는 소듐 채널 유전자의 이상은 증상이 있는 한쪽 부모에서 같이 발견되는 경우도 있지만(10%–15%), 부모에서는 발견되지 않고 증상도 없는 경우가 대부분이다.2 그렇다면 부모에서 발견되지 않는 환자의 유전자 이상은 어디에서 왔을까? 부모의 유전정보는 난자와 정자라는 생식세포에 저장되어 있고, 수정과정을 거쳐서 태아에게 전달되게 된다. 뇌, 심장, 간, 신장 세포 등 부모의 모든 세포에 저장되어 있는 유전정보는 태어날 때 가지고 있는 상태로 평생을 지속하는 것이 아니라, 세포분열을 할 때마다 크고 작은 유전자 이상(돌연변이)이 발생하게 된다. 난자와 정자와 같은 생식세포도 많은 세포분열 과정을 거치면서 돌연변이가 발생하는데, 생식세포 수준에서만 발생한 돌연변이는 부모에서는 증상을 일으키지 않지만, 태아에게 전달되었을 경우에는 전혀 다른 결과를 가져오게 된다. 그 이유는 태아에게 전달된, 돌연변이가 발생한 생식세포로부터 태아의 모든 조직과 장기가 만들어지기 때문이다. 이러한 형태를 de novo 돌연변이라고 부른다.

이러한 형태의 돌연변이에 의한 질환은 증상의 중증도가 심해서 후손에 전달이 어렵고 따라서 가족적(familial) 형태가 아닌 산발적(sporadic) 형태로 나타나는 경우가 많다. 최근 유전자 검사 기술이 발전하면서 지적 장애, 뇌전증, 자폐증, 조현병 등의 신경계 질환의 일정 부분이 de novo 돌연변이에 의해서 발생하는 것이 밝혀지고 있다.3-5 De novo 돌연변이의 개념은 유전성 신경질환을 이해하는 데 매우 중요하고 최근 유전학 연구 분야에 중심이 되고 있다. 하지만 오랫동안 유전이라 하면 “genetic”보다는 “hereditary”의 개념으로 받아들여 왔다.

뇌전증의 유전원인: 역사적 고찰

고대로부터 뇌전증은 악령이나 마귀에 씌워 증상이 나타나는 정신질환으로 생각했지만 의사, 학자들은 뇌전증은 초자연적인 증상이 아닌 기질적 원인이 존재하는 신경질환일 가능성을 항상 염두에 두고 있었다. 고대의 히포크라테스로부터 Sieveking (1816–1904), Reynolds (1828–1896), Gowers (1845–1915), 그리고 현대의 Lennox (1884–1960)에 이르기까지, 유전(hereditary)을 뇌전증의 중요한 원인으로 생각하였다.6-8 물론 당시에는 현재와 같은 유전학의 개념이 정립되어 있지 않았고, 뇌 영상이나 혈액 검사 등의 다른 원인을 규명할 만한 수단이 없었던 시기였음을 고려할 필요가 있다. 그렇다면 유전자 검사나 다른 원인을 증명할 수 있는 수단이 부족하였음에도 불구하고 어떻게 유전 원인을 생각하게 되었을까 하는 의문이 드는데, 다음과 같은 내용들을 근거로 추론해 볼 수 있다. 첫 번째로, 많은 수의 뇌전증 환자들이 뇌전증 그 자체 이외에는 다른 원인과 연결할 만한 증상과 소견이 없음을 발견하였고, 이를 Sieveking은 ‘소인(predisposition)’, Reynolds는 ‘특발성(idiopathic)’이라고 표현하였다.9 특발성을 ‘환자가 가지고 태어나는 내부적 요인 혹은 질환 그 자체(internal cause or morbus per se)’라고 해석하면서 유전 요인과 연결을 하였는데, 특발성이란 용어는 최근까지 뇌전증의 분류체계에 채택되어 사용되었다. 둘째로, 뇌전증은 종종 다른 신경·정신질환과 함께 발생하거나 함께 대물림되는 것을 발견하였는데, 이를 neurological taint, neuropathic trait, neuropathic tendency로 표현하였다. 이는 현재의 지적 장애, 자폐, 정신질환 등 뇌전증과 동반되는 질환(comorbidity)의 개념과 거의 유사하며, 실제 최근의 연구 결과에서도 동반질환과 뇌전증의 유전 원인 스펙트럼이 상당 부분 겹친다는 사실이 증명되고 있다.10 셋째로, 기질적인 원인이 없어 특발성으로 분류되는 뇌전증 환자에서 가족력이 더 흔하게 나타나는 것을 관찰하였다. 이는 Lennox11가 진료하던 환자를 대상으로 발표한 연구결과에 잘 나타나 있다(Fig. 1). Lennox는 유전 원인이 근원을 차지하고 있고, 일부 유발 요인이 추가되면 일정 수위를 넘어서 증상이 나타나는 저수지(reservoir)모형을 고안하였고, 유전 원인과 환경 요인과의 상호작용을 나타내는 강의 지류를 이용해서 설명하기도 하였다(Fig. 2). 위와 같이 임상적인 관찰을 통해 유전 원인에 대한 개념이 근대 학자들에 의해 형성되었으며, 현재의 개념과 대부분 일치한다고 평가할 수 있다.12-14

유전 원인의 빈도 및 뇌전증 분류체계

1975년 Epilepsia에 발표되었던 Minnesota주 Rochester 지방의 역학연구 결과를 보면 약 75%의 뇌전증 환자의 원인을 특발성으로 분류하였다.12 최근까지 뇌영상 기술이 발전하면서 구조 이상을 발견하는 빈도가 늘어났고, 자가면역 기전에 의한 원인이 새로 추가되었으나 상대 비율은 아직까지 큰 차이가 없는 것으로 파악하고 있다(Fig. 3).13

여기에서 국제항뇌전증연맹에서 뇌전증의 원인에 대한 분류를 위해 사용하였던 용어를 정리해 볼 필요가 있다. 1989년 이후 뇌전증의 원인 분류에 사용하였던 용어는 특발성, 증상성(symptomatic), 잠재성(cryptogenic)이었다.15 이미 잘 알려진 중추신경계의 질환이 원인이 되는 증상성과 달리 특발성과 잠재성의 구분은 혼선을 가져오는 경우가 많았다. 1989년 분류에서는 과거부터 타고난 유전적 소인(hereditary predisposition)이 원인일 것으로 생각했던 특발성이라는 용어를, 증상성일 가능성이 높지만 원인을 모르는 잠재성과 구분하려고 하였다. 하지만, 청소년 근간대뇌전증(juvenile myoclonic epilepsy)과 같은 특발성 전신뇌전증(idiopathic generalized epilepsy)과 중심측두극파를 동반한 양성 소아뇌전증(benign childhood epilepsy with centrotemporal spikes)과 같은 특발성 부분뇌전증(idiopathic focal epilepsy)에서는 최근 발전된 유전학 검사기법에도 불구하고 정확한 유전 원인과 기전을 찾아내지 못하고 있는 반면, 발달지연과 지적 장애 등이 동반되어 잠재성으로 구분하였던 그룹에서는 다양한 원인 유전자가 발견되고 있다.16

또한, 선천성 피질기형의 일부에서도 유전 원인이 발견되고, 진행성 근간대뇌전증(progressive myoclonic epilepsy) 같은 대사성 원인으로 분류되는 뇌전증에서도 유전 원인이 발견되는 빈도가 증가하고 있다.17-19 2011년에 발표된 국제항뇌전증연맹 보고서에 의하면 특발성을 유전성(genetic)으로 잠재성에 해당하는 부분을 원인 미상(unknown)으로 새롭게 분류하였다.20 2017년에는 기존의 구조적(structural) 및 대사성(metabolic) 원인에 감염성(infectious) 및 면역(immune) 원인을 추가하였다(Fig. 4).21 과거로부터 유전 원인으로 생각하였던 특발성 뇌전증은 유전성(genetic)으로 분류를 하고 있음에도 현대의 발전된 유전검사 기법으로 유전 양식이나 기전에 대해 아직 구체적인 결과를 내지 못하고 있는 반면, 이전에 증상성, 잠재성이라 생각했던 그룹에서는 단일 원인 유전자가 새롭게 규명되는 빈도가 증가하고 있다. 따라서, 향후 유전 원인이 얼마나 될 것인가라는 질문보다는 규명할 수 있는 유전 원인은 얼마나 되는가에 대한 논의가 더 필요하고, 유전 원인에 대한 용어와 분류체계의 변화도 주의 깊게 살펴볼 필요가 있다.

결론

뇌전증의 원인이 다양한 반면 상당수의 환자에서 뇌의 이상을 밝힐 수 없다는 사실은 본격적인 유전학이 발전하기 이전부터도 잘 인지되었던 사실이다. 가족 안에서 비슷한 환자가 발견되고, 다른 신경학적 동반질환이 같이 발생하는 것을 관찰하면서 유전 원인에 대한 개념이 형성되었다고 볼 수 있다. 순수한 유전 원인을 가질 것으로 생각하는 환자군을 특발성이라 부르고 현대의 뇌전증 분류체계에 사용하였지만, 특발성 뇌전증은 현대 유전학으로도 아직 명확한 유전 원인과 기전을 밝혀내지 못하고 있다. 최근의 발전된 유전 검사기법을 이용하여 de novo 돌연변이가 유전성 뇌전증의 중요한 기전으로 밝혀지고 있지만, 전체 뇌전증에서 차지하는 부분은 아직 크지 않다. 과거의 분류체계였던 특발성, 증상성, 잠재성 모두에서 유전 원인이 확인될 수 있어, 유전 원인을 효과적으로 포함할 수 있는 분류체계에 대한 연구가 필요하다.
