Abstract

Cognitive impairment is prevalent in epilepsy. Studies have revealed that up to 70%–80% of epilepsy patients have various degrees of cognitive impairment in attention, executive function, language function, and memory. Factors associated with cognitive impairment include, but are not limited to, genetic predisposition; medications (including antiepileptic drugs); structural lesions; degenerative processes; mood disorders; type, frequency and severity of seizures; age of onset; and duration of epilepsy. Antiepileptic drugs, including barbiturates and topiramate, are known to affect cognitive function. However, cognitive impairment is frequently noticed at the onset of epilepsy and antiepileptic drugs usually are not independently associated with cognitive dysfunction. In addition, caution is needed that other factors associated with cognitive impairment does not necessarily entail a causal association. The existence of a common pathomechanism underlying both cognitive impairment and epilepsy should be considered. Vascular insult and tau pathology may underlie both cognitive impairment and epilepsy. Surgery could be a good treatment option for refractory epilepsy. Potential cognitive adverse events following the surgical removal of brain tissue are limited and could be outweighed by succesful seizure control and improved quality of life.

서론

뇌전증은 전 세계 약 5천만 명의 인구가 앓고 있는 질환으로, 최근 시행된 메타연구에 따르면 뇌전증의 평생 유병률은 인구 천 명당 7.60명(95% 신뢰구간, 6.17–9.38), 발생률은 10만 인년당 61.44명(95% 신뢰구간, 50.75–74.38)으로 확인되었다.1 또한, 활동성 뇌전증 환자의 대략 50%가 최소 한 가지 이상의 동반 질환을 가지고 있으며,2,3 이는 일반 인구와 비교하여 약 8배 더 높은 수준이다.4,5 특히, 인지장애는 기분 및 행동장애와 함께 뇌전증의 흔한 동반 질환으로 만성 뇌전증 환자의 70%–80%에서 다양한 수준의 인지장애를 동반하는 것으로 알려져 있다.6 뇌전증에 있어 인지장애와 연관된 요인으로는 뇌전증의 유형, 뇌전증 발생의 기저 원인, 발병 연령, 발작 빈도, 뇌전증의 유병기간 및 치료의 영향 등이 있으며, 개개인의 인지적 예비능(cognitive reserve capacity)에 의해서도 인지장애의 정도가 좌우될 수 있다. 본 종설에서는 뇌전증 환자에서 초기 인지기능 장애 및 나이에 따른 경과, 고령에서의 뇌전증과 인지장애, 인지장애에 있어 약물 및 수술의 영향에 대해 논의해보고자 한다.

본론

1. 뇌전증 초기 인지기능 및 나이에 따른 경과

연구에 따르면 인지기능의 저하는 뇌전증 발작 당시에 이미 동반되어 있는 경우가 빈번하다.7,8 주의력 및 집행기능(executive function)의 장애 및 기억력 장애가 최대 70%에서 보고되고 있으며, 저학력, 기질적 뇌 병변 및 전신성 강직-간대성 발작이 동반된 경우 더 낮은 인지기능을 보이는 경향이 있다고 보고되어 있다.9 뇌전증 환자에서 인지기능의 저하가 동반된 경우 뇌전증 발작에 따른 인지기능 저하나 약물 사용에 따른 부작용으로 치부하는 경우가 있으나, 뇌전증 초기부터 인지기능의 장애가 동반되는 경우가 많다는 점은 이러한 판단에 주의가 필요함을 시사한다.6 뇌전증 환자에서 인지기능 저하 동반 시 뇌 구조적 병변, 뇌염증, 유전적 소인 및 뇌 퇴행성 변화 등 뇌전증 및 인지장애와 모두 연관된 기저 병리기전에 대한 고려가 필요하다. 또한, 새롭게 뇌전증으로 진단된 환자의 경우 인지기능 장애에 대한 고려 및 관련 평가가 필요하다.
어린 연령에서 뇌전증이 발생할수록 인지기능의 장애가 동반될 가능성이 높으며, 특히 항뇌전증약에 반응하지 않는 뇌전증을 가진 경우 발병 연령과 인지기능 간에 유의한 연관성을 가지는 것으로 확인되었다.10,11 나이에 따른 경과에 대해서는 6세에서 68세 사이의 연령별로 만성 측두엽뇌전증 환자와 정상인의 레이 언어학습 검사(Rey Auditory Verbal Learning Test) 시행 결과를 비교한 연구가 있으며, 뇌전증 환자군에서 정상인에 비해 유의하게 점수가 낮았으나 연령별 감소 정도는 두 군에서 비슷하여 감소 추세가 비교적 평행하게 나타났다.12 14세에서 60세 사이의 만성 측두엽뇌전증 환자 및 정상인을 대상으로 한 자기공명영상을 통한 구조적 연구에서도, 초기 뇌 부피, 두께 및 표면적의 차이가 전 영역에서 고루 나타났으나 나이에 따른 감소율은 크게 차이가 없어 정상군과 만성 측두엽뇌전증 환자군에서의 감소가 비교적 평행하게 진행하는 것이 확인되었다.13 다만, 가쪽 뇌실(lateral ventricle) 및 제3뇌실(third ventricle)의 부피 증가 속도는 만성 측두엽뇌전증 환자에서 유의하게 더 높은 것이 확인되었다. 따라서, 소아청소년기 뇌전증의 발생이 인지기능의 장애에 영향을 미치거나 인지기능의 장애와 함께 동반될 수 있으나 일반적으로 뇌전증의 유병기간이 길어짐에 따라 정상인에 비해 인지기능이 점점 더 나빠진다고 하기는 어렵다고 할 수 있으며, 인지 기능 악화시 인지기능을 초래하는 다른 원인에 대한 고려가 함께 필요하다.

2. 고령에서의 뇌전증과 인지장애

뇌전증은 아동 및 청소년기 발병과 함께 50대 이후 연령층에서 발병 및 유병률이 다시 증가하는 경향을 보인다.14 고령에서 발생하는 뇌전증의 경우 약 2/3에서는 뇌졸중이나 종양 등 뚜렷한 구조적 병변과 연관되어 발생하나, 1/4 이상에서는 치매를 포함한 인지기능의 장애나 알 수 없는 이유에 의해 발병하는 것으로 알려져 있다.15 특히, 치매는 뇌졸중과 더불어 뇌전증의 가장 흔한 동반질환으로, 치매가 있는 경우 뇌전증 발생 위험도가 약 2.5배 증가하며 반대로 고령에서 발생한 뇌전증 환자에서 알츠하이머 치매의 발생 위험도가 약 3배 증가한다는 보고들이 있다.16-18 또한, 알츠하이머 치매가 있는 환자의 약 40%에서 뇌전증양 발작파(epileptiform discharge)가 측두엽 또는 전측두엽에서 관찰되며, 뇌전증을 동반한 경우 인지기능의 장애가 더 심한 것으로 보고되고 있다.19-21 따라서, 고령에서 뇌전증과 치매와 연관성이 매우 높음을 알 수 있으며, 둘 간의 공통된 발병기전으로는 혈관성 병리와 함께 알츠하이머 치매에서 잘 알려진 베타 아밀로이드(amyloid β) 침착 및 타우 병리(tau pathology)가 제시되고 있다.22 노인에서 뇌전증이 비교적 흔함에도 불구하고 상대적으로 덜 인식되어 있는데, 이는 측두엽뇌전증 비중이 높은데다 비운동성 국소발작(focal onset non-motor seizure)으로 나타나 인지를 잘 못하거나 뇌전증 발작으로 인한 인지장애가 경도 인지장애나 치매 또는 섬망으로 오인되고 또한 노인 뇌전증에서 치매 역시 병발하기 때문일 가능성이 높다. 뇌전증지속상태(status epilepticus)가 노인에서 더욱 빈번하며, 뇌전증을 새로 진단받은 노인 환자의 상당수가 처음부터 뇌전증지속상태로 내원하는데 이는 기존의 뇌전증 발작이 노인에서 잘 인식되지 못하다가 뇌전증지속상태가 나타나고서야 인식되는 것이 원인일 가능성이 있다.23,24 따라서, 노인에서 인지장애를 보인 경우 뇌전증을 같이 의심할 필요가 있다.

3. 약물 치료와 인지장애

항뇌전증약 역시 부분적으로 인지기능 장애에 영향을 줄 수 있다. 이는 phenobarbital 및 benzodiazepine 계열 약물들을 비롯 기존의 뇌전증 약에서 더 두드러지며 새로 개발된 항뇌전증 약들에서는 비교적 인지기능의 장애가 적은 경향이 있으나, topiramate의 경우 일부 환자에서 부작용으로 단어 찾기 어려움(word finding difficulty) 및 언어유창성(fluency) 등의 표현성 언어장애가 두드러지게 나타나는 경우가 있다.25-27 또한, 항뇌전증약의 빠른 증량 및 고용량의 뇌전증 약물 사용, 높은 혈중 약물농도와 함께 동시에 사용하는 항뇌전증약의 개수가 증가할수록 인지기능 관련 부작용의 위험도가 증가한다.25 특히, 어린이나 고령층의 경우 약동학적 또는 약력학적 요인에 의한 약물 부작용에 더욱 취약하다.28 또한, 어린 연령에서 동반하는 인지기능의 장애는 뇌 발달 및 학습능력에 영향을 줄 수 있으므로 주의를 요한다.29 고령에서 발생하는 뇌전증의 경우 상기 기술한 대로 인지장애를 같이 동반할 가능성이 높다는 점을 고려하여 항뇌전증 약물 치료에 따른 추가적인 인지 저하의 우려가 낮은 것으로 알려진 lamotrigine, levetiracetam 등의 약물 사용을 고려하는 것이 좋다.25,26
그러나, 뇌전증 환자의 인지장애의 요인은 다양하며 환자의 전반적인 신체 및 정신상태에도 영향을 받기 때문에 많은 경우 항뇌전증약의 영향만을 따로 분리하기 어려우며, 항뇌전증약 사용으로 인한 발작 조절의 대가로 인지기능 저하를 감수한다는 인식 역시 항뇌전증 약물의 치료를 통한 발작 조절이 인지기능 및 정신상태에 긍정적인 영향을 주는 부분을 같이 고려할 때 바람직하지 않다.30,31 따라서 뇌전증 환자에서 인지장애가 동반된 경우 환자별 임상 소견을 종합적으로 고려하여 판단할 필요가 있다.

4. 뇌전증 수술 후 인지기능

뇌전증의 치료를 위하여 일반적으로 항뇌전증약을 사용하나 2제 이상의 항뇌전증약에도 조절되지 않는 경우에는 추가적인 약물 치료를 통해 조절될 가능성이 5% 미만이다.32 난치성 뇌전증에서 수술적 제거의 경우 보고에 따라 다양하나 대략적으로 2/3 정도의 환자에서 발작 감소의 효과가 있으며, 약 40% 정도에서는 발작 관해(seizure-free)에 성공하고 소아의 경우 더욱 성공률이 높은 것으로 알려져 있다.33,34 뇌전증 수술은 기본적으로 뇌 조직을 절제하거나 뇌 조직에 손상을 가하게 되므로 이에 따른 인기기능 저하의 위험이 있다. 대략적으로 1/3에서 1/4의 환자들에게 일정 정도의 기억력 장애가 나타나는 것으로 보고하고 있다.35,36 측두엽뇌전증에서 좌측 측두엽 절제의 경우 언어기억(verbal memory)의 장애, 우측 측두엽 절제의 경우 공간기억(spatial memory)의 장애가 생길 수 있다.35,37 또한, 수술 후 기억력의 장애는 수술 전 평가에서 높은 수준을 보일수록 더 감소하는 경향이 있으며, 소아의 경우 수술 후 인지기능 회복에 더욱 유리한 것으로 알려져 있다.38-40 장기간의 추적 연구에서 인지기능은 좌측 측두엽 수술 후 최대 2년간 감소를 보일 수 있으나 이후로는 유지되는 양상이었으며, 우측 측두엽의 경우 초기 인지기능의 호전을 보이기도 하나 2년 후부터는 수술 전과 비슷한 수준으로 유지되었다.39,41 수술 후 2년에서 10년 사이의 장기적 예후를 본 연구에서도 역시 측두엽 절제 후 2년 이후에는 비교적 안정적인 인지기능을 유지하였다.42 또한, 10년 뒤 직업과 관련한 예후를 본 평가에서 뇌전증 수술 시 절제 위치(좌우측) 및 수술 후 발생한 언어기억의 장애는 직업 유지와 유의한 관련성이 없었으며, 수술 후 발작 소실(seizure freedom)의 성공 여부가 중요하게 작용하는 것으로 확인되었다.43 뇌전증 후 삶의 질을 연구한 다른 연구에서는 발작 소실에 이른 경우 인지장애 후유증에 관계없이 삶의 질이 개선되나, 그렇지 않은 경우 기억력 장애 저하가 같이 동반된 경우엔 삶의 질도 떨어질 수 있다는 결과를 보여주었다.44 소아를 대상으로 한 연구에서 측두엽뇌전증 수술 후 남아있는 해마의 용적이 수술 후 언어기억 기능과 관련이 있으며, 특히 좌측을 수술한 경우 더 두드러졌고, 절제한 용적 및 측두극(temporal pole)의 보전 정도가 수술 후 의미기억(semantic memory) 기능과 관련 있는 것으로 확인되었다. 따라서, 내측두엽 뇌전증에서 전측두엽 절제술(anterior temporal lobectomy)이 표준 치료로 되어 있으나, 실제 절제 범위를 축소할 경우 인지장애 측면에서 더 유리할 수 있음을 시사한다. 고주파열응고술(radiofrequency thermocoagulation)을 통한 정위절제수술(stereotactic ablative surgery), 또는 감마나이프 수술(gamma knife surgery) 등이 대안이 될 수 있으나 이에 대해서는 추가적 연구가 필요하다. 또한, 발작 병소를 충분히 제거하지 못할 경우 발작 조절 실패 및 재수술 등의 위험이 있으며, 감마나이프의 수술의 경우 방사선 괴사(radiation necrosis) 등의 장기 합병증이 발생할 수 있다.45

결론

뇌전증에서 인지기능의 장애가 흔히 동반되나 종종 이러한 부분이 과소 평가되거나 약물이나 만성적 뇌전증에 따른 것으로 치부되는 경우가 있다. 조절되지 않는 발작이나 뇌전증 치료를 위해 사용하는 약물이 인지기능에 영향을 주는 부분이 있으나, 많은 수에서 뇌전증 초기부터 인지기능의 저하가 함께 동반되기 때문에 이들이 단독으로 인지기능 저하의 원인으로 보기는 어려운 경우가 많으며 기분장애 및 인지기능을 동반할 수 있는 기질적 병인 등에 대한 폭넓은 고려가 필요하다. 뇌전증 약물 및 뇌전증 수술 모두 인지기능을 일부 저해할 수 있는 가능성이 존재한다. 그러나, 효과적인 치료를 통해 결과적으로 얻을 수 있는 인지적 및 비인지적 이득이 훨씬 크므로 이들 부작용을 우려하여 치료를 회피할 필요가 없으며 환자의 개별적 상황에 맞는 적절하고도 적극적인 치료가 필요하다. 또한, 노인의 경우 뇌전증이 비교적 흔하므로 인지장애가 동반된 노인에서 뇌전증에 대한 의심 및 감별이 필요하며, 적절한 평가와 함께 뇌전증 진단 시 적절한 치료가 병행될 필요가 있다.
