Status epilepticus (SE), an increasingly recognized neurological issue in the clinical setting, is the second-most frequent life-threatening neurological emergency following stroke. SE has a high mortality rate that is largely contingent on the duration of SE before initial treatment, the etiology of SE, and the patient’s age. SE can be divided into four stages: (1) impending SE, (2) established SE, (3) refractory SE, and (4) super-refractory SE. Different therapeutic strategies are applied in each stage. This review provides up-to-date information on the pharmacotherapy of SE. The treatment of SE is evolving as new medications have become available. Three new preparations—fosphenytoin, rectal diazepam, and parenteral valproate—have implications for the management of SE. However, randomized controlled trials have shown that benzodiazepines should be the initial pharmacological therapy in patients with SE, and despite the paucity of clinical trials comparing medication regimens for acute seizures, there is a broad consensus that immediate diagnosis and treatment are necessary to reduce the morbidity and mortality of this condition. Recent data suggest that midazolam and propofol may substitute for phenobarbital or other long-acting barbiturates to reduce the duration of sedation. Nonconvulsive SE is diagnosed with increasing frequency, as electroencephalographic (EEG) monitoring is performed more often in various clinical situations. In patients with persistent alteration of consciousness with no clear etiology, physicians should obtain an EEG rapidly to identify potential SE. Physicians should rely on a standardized protocol for the management of SE to improve the care for this neurological emergency.
뇌전증지속상태(status epilepticus, SE)는 뇌졸중에 이어 두 번째로 흔한 신경과적 응급상황이다.
141명의 어린이를 대상으로 한 연구에 따르면 lorazepam 비강내 투여가 lorazepam 정맥투여에 비해 투여 10분 이내의 발작 소실 측면에서 열등하지 않았다.
SE의 치료는 환자의 기도확보, 산소공급 및 정맥 혈관 확보(ABC-airway, breathing and circulation)로 시작한다. 특히 SE는 호흡유지가 중요하다. 그 이유는 흥분되고 대사가 증가된 뇌에 경도의 저산소증이 쉽게 뇌손상을 야기할 수 있고 항경련제가 호흡부전을 초래할 수 있기 때문이다. 따라서 기관내 삽관이 필요한 경우가 많고 특히 benzodiazepine과 barbiturate가 함께 사용되는 경우나 마취제가 사용되는 경우는 꼭 필요하다. 만일 경련으로 삽입이 곤란한 경우는 속효성 근이완제(short-acting muscle relaxant)를 사용하는 것이 좋다. 눈으로 보이는 경련이 소실되었다고 하더라도 뇌파상의 뇌전증모양방전에 의한 신경흥분성 신경손상과 난치성 SE로의 발전 가능성이 항상 있으므로 특히 근이완제를 사용하는 경우는 지속적인 뇌파검사가 필요하다. SE 동안에는 여러 생리현상들이 변화하는데 일반적으로 SE 초기에는 혈압이 오르다가 15-30분이 경과하면 혈압이 감소한다. 이때는 뇌의 자동성도 상실되므로 혈압이 뇌혈류의 유일한 결정요소로 작용한다.
SE에서는 다양하고 많은 양의 약물투여가 불가피하여 정맥주사 경로를 확보하여야 하며, 이때 환자의 발작의 종류를 파악하여야 한다. 즉, GCSE인지 복합부분뇌전증지속상태(Complex partial SE, CPSE)를 포함한 비경련뇌전증지속상태(Non-convulsive SE, NCSE)인지를 구분하는 것이 예후 및 치료의 응급여부를 결정하는데 중요하다(
뇌파감시(EEG monitoring)는SE의 진단과 치료에 필수적이나 불가능한 경우라면 적어도 발작이 중단되었지만 의식이 회복되지 않는 경우, 주기편측뇌전증모양방전(periodic lateralized epileptiform discharge, PLED)을 포함한 모든 발작활동(ictal activity)의 중단 후 적어도 30분 이상은 증상이 나타나지 않음을 확인하여야 한다. 1998년의 한 연구에서는 치료로 발작이 중단된 환자의 20%에서 뇌파상의 지속적인 뇌전증모양방전이 관찰되었다.
Impending SE에서의 항경련제의 투여는 기존의 방법으로는 정맥투여를 원칙으로 한다. 현재까지의 항-SE 약물은 크게 benzodiazepine이나 barbiturate로 대표되는 GABAergic drug과 phenytoin과 같은 Na+-channel blocker가 주로 사용되었다. Benzodiazepine은 SE의 강력하고 효과적인 치료제로 gamma-aminobutyric acid (GABA)-A 수용체에 작용하여 염소 이온의 전도를 높여 억제성 신경전달을 항진시킨다. Lorazepam, midazolam, clonazepam, 혹은 diazepam 정맥투여가 흔히 사용되나 lorazepam이 일차약으로 추천되며 반복투여도 가능하다. Lorazepam은 다른 약제에 비해 지질 용해성이 덜하여 말초조직으로의 체내 재분비가 적다는 장점이 있다. 이는 높은 지질친화성을 보이는 diazepam과 대비된다. 이 중 국내에서는 사용되지 않는 정맥내 clonazepam의 경우 Alvarez 등의 연구에서 lorazepam 정맥 투여 보다 refractory SE의 발생 억제에 있어 효과적이었으나, 이 연구 대상군에서 lorazepam이 최적 용량보다 적게 투여되었기 때문에 해석에 주의를 요한다.
Benzodiazepine으로도 경련이 지속되는 경우 일단 확립된 뇌전증지속상태(establishedSE)로 판단하는 것이 일반적이다. 1차 치료로서 benzodiazepine에 반응하지 않는 뇌전증지속상태에서는 phenytoin, valproate, phenobarbital, levetiracetam, 혹은 lacosamide의 부하용량을 정맥내로 투여하는 치료가 사용된다. 이들 약제에 대한 무작위 대조군 연구는 아직 미진한 설정이다. 그 중에서 최근 두 개의 무작위 대조군 연구가 있었는데, Muldlamuri 등은 GCSE에서 lorazepam 이후 2차 치료로서 phenytoin과 valproate, levetiracetam을 비교하였다.
다음 단계로는 phenobarbital이 흔히 사용되었으나 최근에는 midazolam이나 propofol 등의 정맥마취제가 흔히 사용된다. phenobarbital은 부하 용량으로 15-20 mg/kg가 일반적이나 난치성(intractable)인 경우 고용량이 사용될 수도 있다. Midazolam이나 propofol 등은 부하 주사 후 뇌파 감시 하에 지속적인 정맥주사를 하며 용량을 조절할 수 있다. 그러나 NCSE에서 마취제를 사용하는 것이 적절한지에 대하여는 아직 논란이 있다.
Refractory SE에서의 주된 치료는 thiopental, pentobarbital, midazolam, propofol과 같은 정맥 마취 약제들이다.
SE가 조절된 후에 모든 환자에서 항경련제를 유지할 것인지에 대하여는 아직 의문에 있다. 최초의 뇌전증이 SE로 발현되었거나 대사성 질환에 의하여 나타나는 SE의 경우 모든 환자에서 예방적인 목적의 항경련제 치료가 필요하지는 않다. 그러나 SE가 기존의 뇌전증환자에서 나타났거나 뇌병변에 의하여 이차적으로 나타났다면 지속적인 항경련제 치료가 필요하다.
비록 대부분의 NCSE에서 심각한 신경세포 손상의 증거는 명확하지 않지만, 일부의 보고에서 나타나는 장기간의 기억장애와 여러 연구에서 보고되는 NCSE 환자의 사망률 증가는 NCSE 역시 GCSE와 유사하게 치료하여야 함을 증명한다. 또한 장기간의 NCSE는 뇌손상 뿐 아니라 두부외상, 감염, 흡인성 폐렴 등의 위험성을 증가시킬 뿐만 아니라 명백한 전신경련뇌전증지속상태(overt GCSE)로 이행될 위험성을 갖고 있으므로 적극적인 치료가 필요하다. 또한 노인에서 NCSE가 흔하고 임상적으로 초기에 적절한 진단이나 치료경과를 판단하기 곤란한 경우가 많아 뇌파감시는 치료에 필수적이다.
이제까지의 보고에 따른 위험인자로는 (1) 장기간의 지속적인 NCSE, (2) 감염, 대사장애, 저혈압 등의 전신적인 요인을 동반한 NCSE, (3) 뇌졸중 등의 국소적인 병변을 동반한 NCSE, (4) EEG상 빠른 뇌전증모양방전을 보이는 NCSE 등이며 이들 환자는 위험한 환자군으로 분류할 수 있다.
소발작뇌전증지속상태(absence SE, ASE)의 경우 benzodiazepine으로 대부분 쉽게 치료되며 기타 barbiturate, ethosuximide 등도 효과적이다. 급성 benzodiazepine금단에 의한 ASE는 benzodiazepine혹은 barbiturate의 단기치료에 잘 반응한다. 장기간의 치료가 필요한 경우 valproic acid가 유용하게 사용되기도 한다.
CPSE는 phenytoin이나 benzodiazepine, barbiturate 정맥주사에 잘 반응하며 carbamazepine 경구약제를 사용할 수도 있다. 그러나 CPSE의 경우 스스로 발작이 멈추는 경우도 있지만 대부분 재발되는 경향이 높고 치료를 중단하게 되는 경우는 많지 않다. 사실 NCSE에서 GCSE와 유사하게 고용량 phenobarbital, midazolam, propofol 혹은 pentobarbital이 필요한 경우는 드물다. 이러한 경우 topiramate나 levetiracetam
NCSE를 얼마나 적극적으로 치료할 것인가는 중요한 문제이다. 정맥마취제의 사용이 NCSE의 치료에 더 유용하기는 하지만 치료제로 인한 부작용은 NCSE치료에서는 중요하게 고려할 점이라고 할 수 있다.
난치성 뇌전증지속상태(Refractory SE, RSE)에서의 중요한 이슈는 SE에서 보조적(adjunctive)으로 사용되는 항경련제의 유용성에 관한 문제다. 기존의 고식적인 항SE치료는 1차 치료제, 2차 치료제, 3차 치료제 등의 개념으로 사용되었다. 그러나 최근의 치료는 항경련제의 복합투여가 초기부터 이루어지고 있으며 대부분의 RSE에서는 이미 4-5가지 이상의 항경련제가 투여된다. 이는 초기의 phenytoin이나 valproic acid 혹은 levetiracetam 등이 유지되는 상황에서 다른 항경련제가 부가적으로 사용되는 경우가 흔하기 때문이다.
임상적으로는 이미 levetiracetam,
현재까지 SE의 약물치료결과는 만족스럽지 못한 상황이며, 뇌파상의 뇌전증모양방전의 소실을 기준으로 하면 모든 1차 약제의 치료효과가 명백한전신경련뇌전증지속상태(overt SE)에서는 70% 이하, 미세전신경련뇌전증지속상태(subtle SE) 환자군에서는 25% 이하의 저조한 성적을 보였다.
SE는 특성상 무작위 대조군 연구가 쉽지 않거나 윤리적으로 부적절할 수 있다. 특히 SE의 단계가 올라갈수록 더욱 그러하다. 따라서 refractory 혹은 super-refractory SE의 치료는 무근거(evidence zero)의 영역이라 불리기도 한다. 최근의 여러 연구들에도 불구하고 약물 치료의 큰 틀을 바꿀 만한 결과는 없었으며, 특히 2차 약제에 대한 무작위 대조군 연구들이 필요한 상황이다. 약물 치료 이외에도 면역 조절 요법, 마그네슘, 저체온 치료 등에 대한 연구가 추가적으로 수행되어야 할 필요가 있다.

After seizure control has been achieved for at least 12 h, the drug dosage should be slowly reduced over a further 12 h.
